The biweekly statutory health insurance rates for the Empire Plan and Prepaid Health Plan (PHP) have been announced by Cornell's Statutory Benefits office and are listed below. A complete listing of all Health Maintenance Organizations (HMOs) can be found in this week's issue of the Cornell Workplace.
New rates will take effect in the Dec. 18 paycheck for overtime pay ineligible employees (OPI, Exempt), and the Dec. 23 paycheck for overtime pay eligible (OPI, Nonexempt) employees.
If you are changing health insurance carriers (option change), your new health insurance will be effective Jan. 2, 1997. Your former insurance carrier will extend through Jan. 1.
The deadline for submitting changes to your health insurance is Dec. 27.

Statutory health insurance informational meeting
There will be another on-campus statutory health insurance informational meeting on Friday, Dec. 6, from 10 a.m. to noon in Bache Auditorium of Malott Hall. Nancy Zinzola from the statutory benefits office will be present to provide information regarding the Empire Plan. Representatives from Prepaid Health Plan (PHP) will be present to review the PHP plan. Cornell employees are urged to attend the informational meetings to discuss their questions and concerns.
